Patient forms
Tired of sitting in the
waiting room? We know how that feels! A lot of time is spent just filling
out the necessary forms. Wouldn't it be great if you could have the forms
ready to go as soon as you walked into Milauskas Eye Institute?
Now you can! In an effort to reduce the long waits and "downtime"
that could be better spent by you as well as the doctor, we've put our forms
online.
These form are in portable document format (.pdf). You'll need the correct
plug-in for your browser to display the forms. If you can't view a form simply
by clicking on it, please visit adobe.com to get the plug-in or to download
the free Acrobat Reader application.
We've made these forms
even easier to fill out by making them interactive. For the longer forms,
like patient history, you'll be able to type your information right into the
document itself, or use the pull-down menus for simple yes or no questions.
Then, just hit the print button on the form. The printed document will be
filled out and you'll be able to bring it with you when you visit our office!
